Executive Rehab: Private Drug and Alcohol Treatment for Professionals

Quick answer

Executive rehab is a private residential addiction treatment program designed for professionals and business leaders. It typically adds strict confidentiality, private accommodations and scheduled phone or laptop time to evidence-based care. Federal law, including 42 CFR Part 2 and the FMLA, offers important privacy and job protections, and licensed professionals can often use confidential health programs.

What is executive rehab?

Executive drug rehab is a form of luxury rehab built around the realities of a demanding career. People who run companies, lead teams, practice medicine or law, or hold public roles often delay treatment because they fear losing control of their work or their reputation. Executive programs try to remove those barriers.

The clinical core is the same as any quality residential program: medical assessment, supervised detox when needed, evidence-based therapy, medication when appropriate and treatment for co-occurring mental health conditions. The difference is the structure around it.

Wondering whether an executive program fits your schedule and coverage? Call (877) 847-2855 to talk through options and check your insurance benefits. It’s free and confidential.

What does executive rehab typically include?

Features vary widely, but executive programs commonly offer:

  • Private accommodations, often a private room and bath
  • Scheduled work access, such as set hours for phone, email or laptop use
  • Private workspaces or business-center areas
  • A small census, which means fewer people who might recognize you
  • More individual therapy, often focused on stress, leadership pressure and burnout
  • Concierge intake and travel coordination, including discreet arrival
  • Family and partner sessions that fit around busy schedules

Amenities and policies vary and should be confirmed in writing.

How much work access is healthy?

Work access is a tool, not a right, in most programs. Many clinicians limit devices during detox and the first part of treatment, then allow brief, scheduled check-ins. Constant work can pull your attention away from the reason you’re there.

Approach What it looks like Who it may suit
Full disconnection No devices; a trusted colleague covers duties People whose work is a major stressor or trigger
Scheduled access Set windows for calls and email after early treatment Leaders who must handle essential decisions
Delegated contact Program staff relay urgent messages People who want a firm boundary with work

Before admission, set up coverage at work, an out-of-office plan and a short list of truly urgent issues.

How is my privacy protected in executive rehab?

Confidentiality is often the top concern for executives, and federal law offers strong protections.

42 CFR Part 2

42 CFR Part 2 is a federal regulation that protects the confidentiality of substance use disorder treatment records. According to HHS, it applies to federally assisted programs that provide substance use disorder diagnosis, treatment or referral. These programs generally can’t disclose information that identifies you as having a substance use disorder without your written consent, except in limited situations such as a medical emergency or a qualifying court order.

HHS updated Part 2 in 2024 to align it more closely with HIPAA, with a general compliance date of February 16, 2026. Under the updated rule:

  • You can sign a single consent for treatment, payment and health care operations.
  • Your records generally can’t be used against you in civil, criminal or administrative proceedings without your consent or a court order.
  • You can ask to restrict disclosures to your health plan for services you pay for in full yourself.
  • You can file a complaint with the HHS Office for Civil Rights if you believe your records were shared improperly.

Ask each program whether it’s covered by Part 2 and how it handles consent, records and family communication.

Practical privacy steps

  • Ask about the program’s policies on photos, visitors and staff confidentiality agreements.
  • Decide in advance who may receive information about you, and put it in writing.
  • Consider whether to use insurance or pay privately. Using insurance creates claims records with your health plan, though those are also protected by privacy laws.

Can I take FMLA leave for rehab?

The Family and Medical Leave Act (FMLA) gives eligible employees up to 12 workweeks of unpaid, job-protected leave in a 12-month period for a serious health condition, according to the U.S. Department of Labor. Generally, you’re eligible if your employer is covered, you’ve worked there at least 12 months and you’ve worked at least 1,250 hours in the past year.

Under 29 CFR 825.119, substance use can be a serious health condition, and leave may be taken for treatment by a health care provider. Key limits:

  • Absence because of substance use itself, rather than treatment, isn’t protected.
  • An employer may not take action against you for using FMLA leave for treatment.
  • An employer can still apply an established, nondiscriminatory policy, communicated to all employees, that allows termination for substance use in certain circumstances.

Many states also offer paid family or medical leave, and the Americans with Disabilities Act may protect people in recovery who aren’t currently using illegal drugs. Our guide to rehab and your job covers these protections in more detail. If you own the company, plan for governance and delegation with your board or partners.

What if I hold a professional license?

Doctors, nurses, pharmacists, dentists, lawyers, pilots and other licensed professionals face extra questions about reporting and fitness to practice. Many professions have confidential programs designed to help:

  • Physician health programs (PHPs): Most states have a PHP that offers confidential assessment, treatment referral and monitoring for physicians and, in many states, other health professionals. The Federation of State Physician Health Programs connects these state programs.
  • Nurse programs: Many state nursing boards offer alternative-to-discipline programs for nurses with substance use disorders.
  • Lawyer assistance programs: State bars commonly run confidential lawyer assistance programs.
  • Pilots: The FAA’s HIMS program provides a path for commercial pilots to return to flying after treatment, with ongoing monitoring.

In many states, professionals who self-refer and comply with a monitoring agreement can avoid public discipline, but rules vary by state and profession. Some programs require you to use approved treatment providers. Before choosing an executive program, ask whether it has experience with your profession’s health program, and consider talking to a licensing attorney.

How much does executive rehab cost?

Executive programs are often among the more expensive luxury rehab options. Costs vary widely by location, staffing and length of stay. Insurance may cover the medically necessary clinical portion, but not work amenities, and many programs are out-of-network. Check your private insurance benefits, and see does insurance cover rehab for how out-of-network coverage works.

What should executives look for in a program?

  • State license and Joint Commission or CARF accreditation
  • A medical director and psychiatrist on staff
  • Clear written policies on devices, visitors and confidentiality
  • Experience with professional health programs and return-to-work planning
  • Strong dual-diagnosis care for anxiety, depression and burnout
  • An aftercare plan, such as an intensive outpatient program (IOP) that fits around work

It’s free and confidential.

Frequently asked questions

What is executive rehab?

Executive rehab is a type of luxury residential treatment for professionals who need privacy and some ability to stay connected to work. Programs often offer private rooms, workspaces and scheduled phone or laptop access alongside medical care and therapy. Work policies vary by program and by your clinical needs.

Can I work while in executive drug rehab?

Some executive programs allow limited, scheduled work time, often after the first days or weeks. Most clinicians recommend keeping work to a minimum so treatment can succeed. Ask each program about its device policy before you commit.

Will my employer find out I went to rehab?

Treatment programs covered by 42 CFR Part 2 generally can't share information identifying you as a patient without your written consent, with limited exceptions. If you request FMLA leave, your employer can ask for medical certification, but you don't have to share your full records. Confidentiality rules have exceptions, so ask the program how it protects your information.

Does FMLA cover rehab?

Yes, if you're eligible. Under federal regulations, substance use treatment by a health care provider can qualify as a serious health condition for FMLA leave. FMLA doesn't protect absences caused by substance use itself, and employers can still apply established, nondiscriminatory workplace policies.

Will rehab affect my professional license?

It depends on your profession and state. Many licensing boards work with confidential professional health programs that let doctors, nurses, lawyers and others get treatment and monitoring, often without public discipline if they comply. Talk with your program or a licensing attorney before contacting your board.

Is executive rehab covered by insurance?

Insurance may cover the medically necessary clinical portion of treatment, but not work amenities. Many executive programs are out-of-network, so coverage usually depends on PPO out-of-network benefits. Some people pay privately for added discretion.
